Job Description

Under the direction of the Charge Nurse, is responsible for monitoring patient/resident on a one-on-one basis to ensure they are in an environment that optimizes safety and reduces likelihood of injury. Demonstrates performance reflecting the initiative and responsibility expected of a patient/resident monitor. This includes engaging them and properly guiding them as needed. The Patient/resident Monitor must be courteous and compassionate and treat patient/resident, family members, and staff respectfully. She/he is not responsible for providing direct care, lifting, or transferring. If the patient or resident requires care or feeding, the Monitor must immediately notify a CNA or Nurse. (Certified CNAs providing one-on-one care are allowed to provide direct care as needed).

Roles & Responsibilities
  • Provides one-on-one observation of patient/resident ensuring she/he is in an environment that optimizes safety and reduces likelihood of injury.
  • Maintains awareness and is always receptive to the patient/resident’s needs.
  • Promptly notifies nursing staff regarding their need for direct care, feeding, lifting, or transferring.
  • Demonstrates ability to communicate with the RN, LVN and CNAs any observed changes in patient status and responses.
  • Always courteous and compassionate when interacting with patient/resident.
  • Is respectful and professional with family members and staff.
  • Engages patient/resident and uses proper hand placement when guiding patient.
  • Ensuring a relief Monitor is assigned to the patient/resident before leaving them and signs in and out as required.
  • Demonstrates ability to be flexible, organized and function under stressful situations.
  • Operates equipment safely and correctly.
  • Adheres to dress code, appearance is neat and clean, wears identification while on duty, and reports to work on time as scheduled. Use of the computerized time system correctly.
  • Completes annual education requirements, attends annual review, staff meetings and performs departmental in-service.
  • Maintains regulatory requirements, including all states, including Title 22, federal and other regulations. Always maintains and ensures patient/resident and employee confidentiality.
  • Perform duties in a safety-conscience manner to eliminate or reduce the likelihood of work-related injury or illness. Maintains awareness of staff adherence to safety procedures.
  • Represents the organization in a positive and professional manner.
  • Maintains a good rapport and cooperative relationship with all staff.
  • Complies with all organizational policies regarding ethical business practices.
  • Communicates the mission, ethics, and goals of the facility, as well as the focus statement of the department.
Qualifications & Skills
  • High School graduate or equivalent.
  • BLS certification required within 90 days of hire.
  • Hospital or therapeutic experience preferred.
  • Ability to read and communicate effectively in English.
  • Additional languages preferred.
  • Strong interpersonal skills.
  • Basic computer knowledge.
